At around 20 weeks gestation on December 5, 2023, the family met with Dr. Madeleen Mas, medical director of pediatric cardiology at UHealth Jackson Children’s Care. Multiple exams revealed that baby Joshua had an atrioventricular septal defect, a serious congenital heart defect involving holes between the chambers of the heart.
Dr. Mas explained that Joshua would require surgery around three to four months of age, along with ongoing follow-up care for the rest of his life. Jackie felt reassured knowing that she would be monitored monthly and that Joshua would receive care at Holtz Children’s Hospital immediately after birth.
“Prenatal diagnosis allows families to prepare for major problems and can directly affect surgical outcomes,” said Dr. Mas. The proactive approach would provide Joshua with the best possible prognosis, offering hope amidst uncertainty.
During the third trimester, Dr. Mas organized a tour of Jackson Memorial’s Women’s Hospital and a meeting with Dr. Leonardo Mulinari, chief of pediatric and congenital heart surgery. This allowed the Castro family to feel confident in the team of specialists who would guide Joshua’s care after birth.
On April 10, Jackie gave birth to Joshua, a healthy baby boy who remained in the NICU for 23 days before going home. The family’s relief was short-lived, as Joshua began struggling with breathing just three days after discharge, signaling complications that required immediate attention.
Joshua was admitted to Holtz Children’s PICU, where he was placed on oxygen, monitored closely, and had a PICC line inserted. On May 22, he underwent a procedure to close a patent ductus arteriosus, a persistent opening between the aorta and pulmonary artery.
By early June, an echocardiogram showed excessive blood flow to his lungs, causing labored breathing. Dr. Mulinari performed a pulmonary artery banding to reduce the blood flow, allowing Joshua to strengthen and reach the necessary weight for his atrioventricular septal defect repair.
Over the next four months, Joshua faced recurrent hospitalizations due to COVID-19, fevers, sapovirus, and a blood infection. He received antibiotics, blood transfusions, and a gastric feeding tube to aid in weight gain, demonstrating resilience and determination.
The child life team at Holtz Children’s provided invaluable support for Joshua and Ellie, creating educational tools and a stuffed animal replicating Joshua’s original heartbeat. These interventions helped Ellie understand her brother’s medical journey and maintain a connection during challenging hospital stays.
By September, Joshua had surpassed 5 kilograms, allowing surgery to proceed. On September 17, Dr. Mulinari performed open-heart surgery using cardiopulmonary bypass to oxygenate and circulate blood throughout Joshua’s organs safely.
During surgery, the pulmonary band was removed and a patch was placed to close the holes between the heart chambers. Remarkably, Joshua was extubated the same day, signaling a successful procedure and the beginning of his recovery.
Just five days after surgery, Joshua returned home with his family, demonstrating the extraordinary outcome achieved through a multidisciplinary team approach. Dr. Mas emphasized that despite the complexity of the case, the support of nurses, therapists, and specialists contributed to one of the best outcomes in her 34-year career.
Joshua’s recovery will include follow-up with multiple specialists, including cardiology, immunology, endocrinology, hematology, neurology, genetics, and gastroenterology. He will also participate in outpatient therapy for speech, physical, occupational, and feeding development to ensure ongoing health and independence.
